Add an Activity Report
1. Enter Activity Details
- Select the date and time of the activity.
- Conduct a brief risk assessment and enter your justification.
- Select the person who reported the activity.
- Select the person to whom the activity was reported to.
- Provide a detailed description of the activity
- Complete the checklist questionnaire and make any notes necessary.
- Check the box to indicate that follow-up checks are required.
2. Conduct Follow Up Checks (if required)
- Select the date of the check.
- Select the person or supplier conducting the follow-up check.
- Enter any necessary report details.
- Select the follow-up check result between two options; Clear / Not Clear.
3. Close out Activity Report
Enter any closing commentary and close the record.
