1. Enter Activity Details

Enter Activity Details
  1. Select the date and time of the activity.
  2. Conduct a brief risk assessment and enter your justification.
  3. Select the person who reported the activity.
  4. Select the person to whom the activity was reported.
  5. Provide a detailed description of the activity.
  6. Complete the checklist questionnaire and make any notes necessary.
  7. Check the box to indicate that follow-up checks are required.

2. Conduct Follow Up Checks (if required)

Conduct Follow Up Checks (if required)
  1. Select the date of the check.
  2. Select the person or supplier conducting the follow-up check.
  3. Enter any necessary report details.
  4. Select the follow-up check result between two options: Clear/Not Clear.

3. Close Out Activity Report

Close out Activity Report

Enter any closing commentary and close the record.
